Position Overview: - Job Title: Registered Nurse - Palliative Care
- Location: 2230 Kingwood St, Florence, OR 97439
- Start Date: January 5, 2026
- Duration: 13 weeks
- Schedule: Day shift, 5 days/week, 8-hour shifts (08:00 - 16:30)
- Unit: Hospice RN (Unit 4010)
- Patient Census: 65 patients daily
- Shift Type: Block scheduling
Key Responsibilities: - Support 12-15 hospice families with comprehensive assessments, pain/symptom management, and wound care.
- Utilize EMR systems, including Epic and Remote Client platform.
- Conduct initial patient evaluations and determine eligibility for hospice care.
- Develop, coordinate, and manage individualized care plans.
- Educate and instruct patients and caregivers.
- Coordinate interdisciplinary team care.
- Perform discharge planning and evaluate patient outcomes.
- Document thoroughly and promptly.
- Assist with bereavement services post-death.
- Collaborate with team members for seamless care delivery.
Qualifications & Requirements: - Experience: Minimum of 2 years acute care experience.
- Certifications: BLS (preferred)
- Skills: Symptom assessment, pain management, wound care, medication administration, end-of-life care, cultural awareness, patient/family education.
- EMR: Experience with Epic required.
- Additional Requirements: Must have applied for Washington state license, fully vaccinated (booster not required), references from last two years (including supervisor).
Work & Staffing Details: - Patient Ratios: Days 1:8, Nights 1:1, Weekends 1:2
COVID-19: Vaccination required, exemptions only for medical/religious reasons.\n\nTravel & Licenses: No travel required; must have Washington license application in process.\n\nOther: Must be willing to float to different units as needed; no local radius accepted.
