A mission-driven Managed Long Term Care (MLTC) program serving the diverse communities of Brooklyn is seeking a Registered Nurse to join its Intake team. This is a hybrid role requiring field-based assessments and in-office collaboration.
\nThe MLTC Intake RN plays a vital role in evaluating potential new members for program eligibility, educating them and their families on available services, and supporting a smooth transition into long-term care. Bilingual language skills in Chinese or Spanish are highly preferred due to the populations served.
\n\nKey Responsibilities:
\nConduct in-person assessments using the UAS-NY tool to determine program eligibility and safety for potential members.
\nEducate prospective members and families about the MLTC program, services, and enrollment process.
\nCollaborate with the Intake/Outreach and Clinical teams to coordinate timely, appropriate onboarding of new members.
\nCommunicate professionally with referral sources, physicians, caregivers, and community partners.
\nParticipate in outreach activities to support community engagement and referrals.
\nQualifications:
\nActive New York State RN license required.
\nBachelor’s degree in Nursing required.
\nMinimum 1 year of acute care experience .
\nProficiency in UAS-NY assessments required.
\nExperience in home care and care management preferred.
\nStrong communication and interpersonal skills; fluency in Spanish or Chinese is highly desirable.
\nProficiency in Microsoft Office applications.
