Physical Therapist – Home Health | Job Description
Job Summary:
The Physical Therapist will provide high-quality in-home therapy services to patients, focusing on improving mobility, restoring function, and promoting independence. This role requires strong clinical skills, excellent communication, and the ability to work independently while traveling between patient homes.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
- Deliver therapeutic interventions to improve mobility, strength, and functional outcomes.
- Complete accurate documentation using EPIC Remote Client , Rover , and OASIS .
- Educate patients and families on home exercise programs, safety, and mobility techniques.
- Collaborate with interdisciplinary teams to ensure coordinated, patient-centered care.
- Manage daily travel efficiently while maintaining timely patient visits.
Requirements:
- Experience: Minimum 1 year of skilled Home Health experience required.
- Licensure: Active Physical Therapist (PT) License ; pending license accepted .
- Certifications: BCLS required.
- OASIS experience is mandatory.
- First-time travelers accepted.
Special Requirements:
- Must have reliable personal transportation.
- Travel routes are organized to minimize windshield time; occasional max radius ~60 miles (60–90 minutes/day).
- No weekend or on-call requirements.
- No floating to other units.
- Block scheduling not available.

- Start Date: ASAP
- Shift: 8-hour Days (no 4x10s or 3x12s)
- Patient Ratio: 1:1; up to 6 visits/day (30 productivity points)
- Attire: Business casual; scrubs allowed (no jeans, no required color)
- Charting: EPIC Remote Client & Rover app / OASIS
- Parking: Free
- Mileage Reimbursement: IRS Standard
